Sustainability and Transformation Plan (STP)

The Long Term Plan (LTP) for Shropshire and Telford & Wrekin Sustainability and Transformation Partnership (STP) sets out our ambition for the health and care for our residents over the next five years. It is not only our response to the requirements set out in the national NHS Long Term Plan launched in 2019, but also more importantly sets out our collective ambition to drive local improvement and innovation.

It brings together the ambitious aspirations of all eight partner organisations which include our two Local Authorities, NHS commissioners and providers. It shows how, collaborating together as one, we will bring about the necessary changes to health and care over the next five years for the benefit of local people and the staff delivering their care.

The plan sets out a broad framework. It aims to shape how over the next five years we will deliver the vision for innovative, safe and high-quality care, to support people in their own communities, drive forward efficiency and deliver environmentally friendly services that make the best use of our resources.
