Adult respiratory service

The service sees patients who have a clinical diagnosis of COPD or Bronchiectasis who may be stable or acutely unwell with an exacerbation of their condition and are aged over 18.

Our services

The services we provide are:

  • Respiratory assessments in clinic or home (if housebound)
  • Follow up reviews
  • Admission Avoidance visits
  • Early Supported Discharge
  • Oxygen Assessments
  • Nebuliser Assessments

New patients

Referrals can be made by:

  • All Healthcare professionals such as GPs, social workers or hospital staff
  • Self-referral, by patients already known to the service
  • Relative of patients already known to the service

Services we do not offer

The service is not available to non-respiratory patients or under 18s.

What to bring with you

When attending a clinic appointment or having a domiciliary home visit you will be asked to bring along/provide the following:

  • Inhalers
  • Medication or a list of them
  • Nebulising compressors

Who are we?

We are a team of specialist nurses who provide a service to patients who have a clinical diagnosis of Chronic Obstructive Pulmonary Disease (COPD) or Bronchiectasis.

We support patients with COPD or bronchiectasis in clinics, in their own homes and also support patients to be discharged from hospital following an admitting diagnosis of an exacerbation of COPD or Bronchiectasis.

You may be seen in the community either in a clinic setting or in your home (if housebound). The team consists of:

  • Respiratory Nurse Specialists
  • Health Care Assistants
  • Administration Staff

We are a county wide service who offer domiciliary home visits if appropriate or clinic appointments in the following areas:

  • Bishops castle
  • Bridgnorth
  • Church Stretton
  • Oswestry
  • Ludlow
  • Market Drayton
  • Shrewsbury
  • Telford (Donnington, Madeley & Oakengates)
