School Nursing

How do Care Professionals refer into this service?
A GP who wants to refer into this service is asked to complete the School Nurse referral form and send to appropriate school nurse base which is detailed on the form.

Types of patients regularly seen
School nurses support the individual health needs of children and young people between the ages of 5 – 19 in mainstream Local Authority schools across Shropshire and Telford and Wrekin. We also provide support to parents and carers of school aged-children where appropriate.
School nurses are in a unique position to be able to provide a valuable link between health, education and home.
