Please provide this booking information document to the family along with the blood request form. This provides information to the parent/carer for booking an appointment
Types of patients regularly seen
We accept referrals for children and young people aged between 1 and 18 years
Registered with a Telford and Wrekin GP or the following practice
- Albrighton
- Bridgnorth
- Shifnal and Priorslee
Blood forms marked as routine
Patients not seen by this service
- Children and Young People registered with a Shropshire GP (except Albrighton, Bridgnorth, Shifnal and Priorslee)
- Infants under 1 year
- Adults over the age of 18 years
- Urgent blood requests
