How do Care Professionals refer into this service?
For submitting first or new referrals, it is important that GPs access a professional who has completed our Wheelchair Referrers’ course, or attend one of the half day courses produced by the service. Contact the team using the number on this page for more information. The dedicated referral form which is available to trained referrers should be completed either electronically or posted back to the service.
