Health Visiting
This part of the website has been designed specifically for GPs, Clinical Commissioning groups, care homes and other care professionals who need the service referral information or access to Trust policies and care leaflets.
How do Care Professionals refer into this service?
Referrals can be made by telephone, letter, birth notification, email or through the helpdesk. Parents or carers can make referrals by accessing the advice line for the relevant area.

Types of patients regularly seen
The Health Visiting Service is a universal service with every child/family having access to a health visitor. Every family is provided with contact details of their health visitor's team base so they can contact them during normal working hours. The core business hours for accessing the health visiting service are Monday to Friday 9am to 4pm.
This service is available to all parents or carers of every child 0-5 years of age, whether they have been born or transferred into the county.
Clients are referred into the service by midwifery services, GPs, Children's Centre Services, local authorities, health care professionals, voluntary agencies and by self referral.
