Community Dying Well Fund

Community Dying Well is the first fundraising charity set up and run by a group of Shropshire Community Health NHS Trust health professionals and volunteers. They are passionate about improving end of life care and support for those patients and family and carers in the community who have chosen their home or community hospital as a preferred place of care.
