How do Care Professionals refer into this service?
GPs can refer directly to DAART in their area by using the contact details on this page or via the Care Co-ordination Centre by email Shropcom.oswestrydaartreferrals@nhs.net or 01743 454903.
Types of patients regularly seen
The DAART service provides diagnostic, assessment and referral to treatment for a range of different patients that have a range of conditions - but who do not have a long-term care plan. Patients normally seen by the DAART service across the county are adults living in Shropshire County, the borough of Telford and Wrekin or mid Wales that have a range of acute illnesses or long term conditions.
Patients not seen by this service
- Children (under 18 years old) are not seen by this service
- Patients with long-term, ongoing treatment needs

No. The DAART service does not provide long-term care to patients. It provides diagnostic, assessment and referral to appropriate treatment only.
