Partnership with Commissioners

The Trust’s strategy is informed by our assessment of our commissioners’ requirements for the local health economy, and of the market. It focuses on offering solutions to the particular QIPP challenges of the local health economy, including the pressures of a large and significantly increasing older population and resulting priority to manage unscheduled care. Our wide range of community- experienced clinicians and network of premises and teams across the county put us in a unique position to meet those needs. As a dedicated community organisation, the Trust has a perfect opportunity to transform services to deliver a radical shift to high quality, accessible community-based services, with a particular focus on services for older people and those with long term conditions.

The Trust has therefore set out three strategic service development themes in its Integrated Business Plan, two of which are transformational whilst the other is categorised as service improvement ie

  • Improved and increased care of the frail and elderly within the community
  • Proactive management of long term conditions
  • Service improvement of specialist services, whilst supporting health improvement initiatives for the population, especially our children and young people.
